Claims Procedure (索赔过程):
  1. Claims must be submitted within 365 days from the date of initial diagnosis of unexpected emergency sickness or disease or accidental bodily injury.
  2. Fully complete Claim Form with requested documents (see Claims Documents as below) required.
  3. Claims should be submitted by mail to 21st Century Visitor's Claims, c/o Manulife Financial, P.O. Box 4262, Stn A, Toronto, Ontario, M5W 5T4, Canada.
Important Notes (特别注意事项):
  1. There is a $0, $50, $250 or $500 deductible per claim (depends on plan you purchased and your age) in this insurance.
  2. Claim Form should be signed by both the insured (受保人) and sponsor (付款人、担保人).
  3. Incomplete forms and/or documents will delay your claim.
Claims Documents(索赔所需材料):
When you submit a claim, please enclose the following documents 索赔时:
  1. Copy of your completed application for insurance or your policy confirmation;
  2. Proof of all travel dates of entry into Canada and the USA (airline ticket, passport or visa);
  3. Original itemized medical bills, receipts and invoices;
  4. Proof of payment;
  5. Complete medical and/or hospital records including diagnosis, x-ray, lab or other
    6. diagnostic testing results, which confirms that the treatment was medically necessary; and
  6. Copy of police report (in the case of a Motor Vehicle Accident 如果是交通事故引起的话).
  7. Completed Attending Physician’s Statement and Visitors to Canada Claim Form;
  8. Completed Visitors to Canada Claim Form;
